New Patients – Register Here Now!
How to Register
Forty Willows Surgery is proud to be a safe surgery for everyone in our community.
In recognition of the barriers to healthcare access faced by people in vulnerable circumstances, including migrants, we commit to protecting the human right to health, and will take steps to ensure that everyone in our community may fulfill their entitlement to quality healthcare.
In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services. For more information on Safe Surgeries, click here
If you would like to register with our practice, you’ll need to complete a GMS1 form and patient questionnaire.
We also ask for proof of ID and address so we can make sure we get your details right, and link any existing medical records you’ve got from a previous GP practice, but these are not mandatory and not being able to provide these won’t prevent you registering.
After you have completed and submitted the form you must email us on BRECCG.fortywillowssurgery@nhs.net with two forms of identification.
Acceptable identification includes:
- Passport
- Birth Certificate
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration / mail forwarding letter.
Until we have seen your identification you will not be registered at the practice.
For further information on registering with your GP, please visit the NHS website.
Out of Practice Boundary Area Registration
Where it is clinically appropriate and practical to register, we now accept new registrations from patients who work in the local area but reside outside our registration area. Patients registered this way would not be entitled to a home visit from the practice. However, if you live outside the practice area and need a home visit, please contact NHS 111 in order to be seen by a practice closer to where they live.
For further information about this type of registration, please contact us on 0208 0000000 or please come into the practice.
Registering for Online Services
